REQUIRED FORMS CAN BE SUBMITTED BELOW BY FOLLOWING THESE STEPS:

STEP 1: Enter Your Registration ID Number - From Your Receipt
STEP 2: Enter Camper's Last Name - From Your Receipt OR if you are a Coach that registered your team, enter your last name in.
STEP 3: Click Continue
STEP 4: Click Upload, Select Proper File - On Your Computer, Click Open
STEP 5: Form is Successfully Uploaded - Status Will Indicate the Uploaded Date
